Terms of Service This agreement between * First Name Last Name (hereinafter referred to as “client”) and Mythic Bones LLC (hereinafter referred to as “trainer” or “Mythic Bones”) pertain to the following: Client’s dog(s) name(s): * (hereinafter referred to as “dog(s)”) For good and valuable consideration, the parties agree as follows: Services: Mythic Bones provides Dog Training, Dog Daycare, Dog Boarding, Dog Bathing and Nail Trimming, and Dog Transportation Services (“doggie taxi”). Mythic Bones does NOT provide any professional grooming services at this time, only basic baths and nail trims. In all instances, Mythic Bones agrees to provide humane dog handling with an emphasis on safety. All care provided will be welfare-driven and lack aversive or punitive techniques. The trainer reserves the right to make adjustments to training plans, feeding schedules and requested services if he/she/they feel that it is in the best interest of the dog(s). The client agrees that Mythic Bones will not be held liable for any damages resulting from the transportation of animals. Property and Necessary Care Items: The client agrees to provide any items necessary for the care of the dog(s) including food, medication and bathing essentials such as prescription shampoos or ear cleaners. Client agrees to provide any comfort items at their discretion, such as toys, blankets, t-shirts, towels, or the client’s preferred shampoos, conditioners or ear cleaners. Mythic Bones agrees to return all comfort items at the end of each requested service, as well as any remaining necessary care items, if any. If a comfort item is discovered to not be returned at the time of pickup, Mythic Bones agrees to coordinate with the client to return the item as soon as possible. The client agrees that Mythic Bones will not be held liable for any damage to comfort items caused by the dog(s) during the requested service. In the case of doggie taxi services, the client acknowledges that seat belt leashes and seat covering hammocks are provided by Mythic Bones and that the client is responsible for providing a harness or other equipment (kennels, car seats, etc) necessary for the safety of the dog during transportation. The client agrees that Mythic Bones will not be held liable for any loss or damage resulting from the client’s failure to provide safe equipment for the pet’s transportation. Drop off and Pickup: Dogs must be checked in and out of the care of Mythic Bones LLC during operating hours, which are 8am-6pm. Late pickups and/or early drop off may incur additional fees. If the client does not pick up the dog(s) within the specified time as discussed between client and trainer, we reserve the right to transfer the dog(s) to the trainer’s home, a local kennel or animal shelter, and to charge any additional fees incurred pertaining to the dog’s transportation, veterinary care, and/or equipment necessary for care of the dog, including but not limited to, food, water, and safety equipment such as leashes, collars, harnesses, or containment such as kennels or crates. In the case of doggie taxi services, the client agrees to be prepared with the dog(s) and their belongings at the agreed upon time of taxi arrival for pickup, as well as to be prepared to receive their dog at the agreed upon time of taxi arrival for drop-off. Health and Vaccinations: Client understands a copy of his or her pet's vaccinations, including Rabies, Distemper, Leptospirosis, Parvovirus, Bordetella, and Canine Influenza Vaccines must be provided prior to the date of any service. Should the dog develop kennel cough, or any other illness relating to the effectiveness of vaccinations, upon returning home, the client agrees that Mythic Bones will not be responsible for any veterinary expenses, or other expenses, incurred. Client also agrees to inform the trainer of any illness incurred upon returning home after services have been provided, in order to prevent the spread of disease or infection between dogs. Mythic Bones reserves the right to refuse admission to any dog who appears unwell or has not been vaccinated. Client understands that dogs will be subject to group play with other dogs in an outdoor space, and accept the inherent risks of the environment. Client agrees that Mythic Bones will not be held liable for any fees incurred due to these inherent risks, such as, minor scrapes, cuts or nicks, insect bites, infection due to debris (dirt, sand, etc.), pulled muscles, any other risks inherently associated with the environment. Client understands that the dog will get dirty, and that bathing services are available for an additional fee but are not required. Behavior: Dogs must be socialized and able to interact with other dogs and humans. Mythic Bones reserves the right to refuse admission to any dog who displays aggressive or destructive behavior. If the dog causes damage to Mythic Bones’ facilities or injures another dog or person, the client agrees that Mythic Bones will not be held liable for any damages. Client acknowledges that dog behavior following training sessions (in-home, onsite or otherwise) is not guaranteed and that results are dependent upon the client’s continued practice and the mental well-being of the dog(s). Health Emergencies: The client agrees to provide Mythic Bones with an emergency contact. Client understands that the emergency contact must be willing and able to take full responsibility for care of the dog(s) at any time. In the event of a health emergency, the trainer will contact the client or their provided emergency contact as soon as possible. If the trainer is unable to contact the client or emergency contact, the trainer will then contact the dog’s regular veterinarian utilizing information provided by the client. If the client does not provide their regular veterinarian’s information, the trainer is unable to contact the dog(s)’ regular veterinarian, or it is recommended by the veterinarian in question that the dog(s) be transported to a local emergency veterinarian, the trainer will take the dog(s) to a local emergency veterinarian as listed below for treatment, and the client agrees that Mythic Bones will not be held responsible for any veterinary fees incurred. Emergency Veterinary Facilities whom the dog(s) may be transported to: Blue Pearl Pet Hospital 607 Lumsden Professional Ct Brandon, FL 33511, (813) 571-3303 - Riverview Veterinary Hospital 7459 S US-301 Riverview, FL 33578, (813) 677-3100 - Animal Emergency Clinic 693 W Lumsden Rd Brandon, FL 33511, (813) 684-3013 Media Release: Client consents to photos, videos, and depictions of client and/or dog to be used by Mythic Bones for promotional, marketing and similar use. Booking: Bookings need to be done in writing, and can be made online, via text, or in person. We reserve the right to deny any booking at any time, for any reason. All bookings are subject to availability. Fees and Payment: All service fees are detailed on the Mythic Bones website (www.MythicBones.com) or are available to obtain from the trainer. The specific fees incurred by the client’s request will be detailed in the invoice provided by Mythic Bones. All fees must be paid in full prior to the end of the provided service. Mythic Bones reserves the right to charge additional fees in response to services requested by the client, as well as additional fees incurred by late pickup. Mythic Bones will issue an invoice to the email provided by the client via Quickbooks Online. The client will be required to pay utilizing the payment methods accepted via Quickbooks Online (credit card, apple pay, bank). NOTE: The client’s cancellation of services with less than 24hrs notice subject the client to a cancellation fee totaling up to half of the amount of the service. Client acknowledges that the total amount of the cancellation fee, while not exceeding half of the amount of the requested service, is issued at the trainer’s discretion and client agrees to pay the cancellation fee in full. Liability: Client agrees that all pets are cared for by Mythic Bones without liability on Mythic Bones’ behalf for loss or damage from disease, theft, fire, death, escape, car accident or for other unavoidable causes. Client accepts sole responsibility for any injury caused by the dog(s) while the dog(s) is/are in the care of Mythic Bones, including injury to employees of Mythic Bones, clients of Mythic Bones, and other pets under the care of Mythic Bones, and the client releases Mythic Bones, and its owners, employees, agents, and representatives, of any liability arising from any such injury. Client accepts sole financial responsibility if the dog(s) cause damage to the facilities of Mythic Bones, including damage to the trainers’ vehicles while utilizing doggie taxi services. Mediation: Client and trainer agree to bring concerns, questions or disputes to the attention of the other party directly and promptly. Both parties agree to seek mediation prior to litigation. Both parties agree to resolve disputes privately and needs to be done prior to posting on online platforms. Termination of Services: Mythic Bones reserves the right to terminate services at any time if the trainer feels that it is in the best interest of the dog, other dogs, or Mythic Bones’ staff. Mythic Bones also reserves the right to terminate services if any of these terms and conditions are breached. Amendments: Mythic Bones reserves the right to amend these terms and conditions at any time, and any changes will be posted on our website. The client’s continued use of services after any such changes constitutes the client’s acceptance of the new terms. Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of the State of Florida, and any disputes arising out of them shall be subject to the exclusive jurisdiction of the courts of Hillsborough County. These terms and conditions govern the use of all services provided by Mythic Bones LLC, and by signing below, the client accepts these terms in full.
